Our Facilitated Enrollment Program assists qualified New York State residents who wish to apply for free or low-cost health insurance for children and adults.

Child Health Plus, Family Health Plus or Medicaid coverage includes (but is not limited to) the following services:

Eligibility for adults (ages 19 – 64) in the Medicaid and Family Health Plus programs is determined based upon family size and family income. As of January 1, 2010, the resource test (assets limitations) has been eliminated for Family Health Plus and Medicaid applicants, 19-64.

Eligibility for children (under Child Health Plus) and for pregnant women (under PCAP) is determined regardless of family resources (liquid assets). All pregnant women and most children are eligible regardless of immigration status

Enrollment in a managed care plan is currently mandatory in NYC. Managed care plans provide access to high quality primary and preventive care services. Your facilitator enroller will provide you with information on how to choose a plan that best suits your needs.

When it is time for your enrollment in CHP, FHP or Medicaid to be renewed, one of our facilitator enrollers will be happy to assist you with applications and the renewal process for qualified New York State residents. Please call our office.
